University employees who work full-time in a permanent position are eligible for the University benefits.
Medical Insurance:
Carrier is Mountain State Blue Cross Blue Shield
Enrollment period is the month of hire and coverage begins the first day of the month after enrollment. Example: An employee is hired January 17 their coverage would begin on February 1. Premium is based upon salary and type of coverage (Employee, Employee and Children, or Family).
Changes in existing health care coverage can be made during “open enrollment” which occurs annually in the spring OR due to a qualifying event outside of open enrollment.
Enrollment period is the month of hire and coverage begins the first day of the month after enrollment. There is no cost to the employee for short term disability.
Elimination Period: 14 Days Accident and Sickness
Benefit: 60% of weekly salary to a maximum of $1,250.00 per week for 11 weeks
Enrollment period is the month of hire and coverage begins the first day of the month after enrollment. There is no cost to the employee for long term disability.
Elimination Period: Following the date your insured short term disability ends or 91 days
Benefit: 60% of weekly salary to a maximum of $5,000.00 per month to age 65
Enrollment period is the month of hire and coverage begins the first day of the month after enrollment. There is no cost to the employee for long-term care.
Long term care is the assistance received when someone needs help with two or more Activites of Daily Living or when someone suffers a severe cognitive impairment. This care could be provided in the home, in an assisted living or residential care facility, or in a skilled nursing facility such as a nursing home.
Group Term Life Insurance and Accidental Death and Dismemberment:

Enrollment period is the month of hire and coverage begins the first day of the month after enrollment. There is no cost to the employee for group term life insurance.
The amount of coverage that an employee is eligible for is based upon a schedule of classifications.
Optional Life Insurance:
Carrier is UnumProvident
An employee may choose optional life insurance during the month of employment and coverage beings the first day of the month following enrollment. Premium, which is based on the amount of coverage selected, the age of the employee, and tobacco use status, is paid by the employee.
During the initial enrollment period, any amount in excess of $100,000 requires a statement of health. To enroll or increase the amount of Optional Life Insurance after the initial enrollment period, the employee must complete a statement of health from and be approved for coverage based upon medical evidence.
Supplemental Insurance and Flexible Spending Account:
Carrier is Colonial
An employee may choose supplemental insurance or a flexible spedning account during the month of employment and coverage beings the first day of the month following enrollment.

Benefits-eligible employees are required to participate in the retirement plan after one year of service, every full-time employee must contribute at least three percent (3%) of the employee’s income to the 403 (b) defined contribution retirement plan.
The University will match and employee’s retirement contribution up to five percent (5%) of the employee’s income after the employee’s first year of service in higher education, up to seven and on half percent (7.5%) of the employee’s income after eight (8%) consecutive years of service in higher education, and up to ten percent (10%) of the employee’s income after ten (10) consecutive years of service in higher education.
